Do i need to fill out parent names? Cannot provide their ssn because they dont have any(never been to the US either, leave it blank?) Form also asks for names used in the past. Should I bring my marriage certificate to show the change of name after marriage? The old social security card has my married name and I want to keep it that way. Never had a ssc with my maiden name(got it after marriage)... things I am taking with me: passport, state id,form ss5. Anything else I need? Thanks.

Yeah I added that. I  wanted to confirm if I should bring my marriage certificate because of the maiden name to married name change /if they ask for that?

They shouldn't ask unless you're changing it again.

 

But carrying 1 entra piece of paper just in case shouldn't be too much of a burden either?
