3025? You should have been given it by the doctors when you had your medical, are you sure you don’t have it? It’s nothing to do with the consulate, it’s where you had your medical that you need to get it from so if you didn’t get it at the time you’ll need to get them to send you a copy.

Good luck.

You should have recieved a copy of your DS3025, but this isn't the case for everyone.

The original paperwork should have been in the envelope that you gave to CBP at the POE, so USCIS should have it on file already. If it wasn't in the packet, you will recieve an RFE asking for it, so if you are unable to get a copy of it, you will need to redo your vaccination assessment.

1. It's is a VJ RECOMMENDATION to provide a photocopy of your PERSONAL DS3025 Vaccination Record PHOTOCOPY with your I485 submission ONLY as a REMINDER to your adjudicator to locate your AFILE which WILL have your medical record completed abroad as it was collected at your POE and transferred from DHS to USCIS.

It is not a USCIS REQUIREMENT to provide a photocopy. It is US SUGGESTING it because adjudicators do not always know about the K1 exemption and will request an I693 instead.

If they see your DS3025 photocopy, they will know you've completed your medical already.

2. Your panel physician is required to give you a copy of ONLY your DS3025 Vaccination Record AND XRay CD. They are not required to give you a copy of your ENTIRE medical. Contact them and remind them of THEIR requirement and your eligibility to file a COMPLAINT with the CDC. And if they do not respond (which they will once threatened) file a complaint.

3. Please fill out your VJ timeline in reciprocity. Your statistics help all members while we help YOU personally. A blank one helps no one and that's not fair.
