Questions on the DS-260 form for spouse in India

Hello - My spouse in India does not have marriage certificate or remember the date of birth of her spouse from her 1st marriage, but does have Divorce document.  She can only guess at the age of the Ex, as getting that information from Indian courts and registers is literally impossible.  She never changed her name during 1st marriage, as it only lasted one month.  Is that sufficient?

 

Who initiates a Police Clearance Certificate request?  As in, does the spouse have to request it from the regional Police Department or it has to be requested by the US Visa Department?  And is there a generic FORM or an example the US Visa Department has, that needs to be completed by the regional Police Department?  

 

Unfortunately, unlike here in the US, data record by cities/states is very poor in India, thus creating these types of situations.

Leave unknown parts blank or enter UNKNOWN if possible and if possible include a statement that she doesn't recall the dates of either event.

 

Is her ex-spouse a citizen of India?

Unfortunately, the DS-260 online form does not allow blanks..unless I am wrong?  Her ex-spouse is Indian citizen.

In that case, enter best estimate and include a statement that the dates are estimates and why they are not precise.

The poor data/record keeping is what leads to extended APs for countries like Pakistan and India.
