Has anyone filed N-565 because they lost their Certificate of Naturalization recently?

 

We had couple question while filing N-565. 

 

1. USCIS wants how/when/where we lost the Naturalization Certificate. For us, we don't know. What has other folks put down for this?

2. For uploading supporting documents, is the 2x2 Passport and copy of the original certificate all is needed? They have some verbiage about sending in original documents, but we are not sure which original documents they are referring to. Besides these two.

RoC Timeline:

  1. 12/01/2023 - Application sent to Tempe, AZ lockbox
  2. 12/06/2023 - Application received by USCIS
  3. 12/12/2023 - Check cashed by USCIS
  4. 12/22/2023 - I-797C notice recieved giving access to 48 months extension letter online.

If you knew exactly where and when you lost it, you wouldn't lose it, you would find it there. Did you carry the certificate everywhere with you? If not, it could have been stored at home and you lost it when moving from old place to new place for example. When and where did you last see it, approximately ? Put that date and place in the answer.
