i see alot of you need a i693a , you can do by mail for $10 contact Dr LE ARnold 4534 wastgate blvd #106 austin texas 78745

very professional i lost his phone # but u can call dir assistance to get his # to call for instructions...........now gonna send in hope i dont get medical rfe ...willl keep u all posted sending in aos as ead today (I hope) frday the 13th lol

mailed out aos and ead 7/13/2007

aol and ead packet received per usps 7/16/2007

checks cashed 07/26/07

noa 1 FOR EAD RECEIVED 07/27/07

noa 1 for aos received 07/28/07

biometrics 08/15/07

ead card ordered 09/20/07!!!!

ead card received 10/01/07! finally!

aos case transferred to California Service Center 10/03/07!

received "Welcome to the United States of America I-797 11/05/07!(dated 10/30/07)!

Permanent Resident card received 11/05/07 (dated 10/31/07)! THank u LorD!

Will file for 10 year GC End of July/Early August 2009!

Removal of Conditions:

Mailed I-751 overnight mail to Vermont Service Center 09/10/09

VSC received the I-751 Packet 09/11/09 About 12 pm Local time

Check was cashed as of Sept 16th 2009

NOA Received 09/19/09 dated 09/14/09

I-797 Appointment Notice received 10/01/09 dated 09/25/09 (Biometrics)

Got they 10 year GC approval letter in the mail dated 01/08/10! yehey!

10 GREENCARD RECEIVED 01/20/10

Citizenship.......maybe............or maybe stay a permanent resident .......................

We dropped ours off at 'Arnold's office yesterday afternoon (we're lucky the office is not only in Austin, but close to us) - hope to pick up the papers on Monday. (They're done by one of the nurses there - she is off until Monday.) :thumbs:

It is the vaccination supplement that will be required for AOS. (Used to transfer a foreign fiancé/spouse's vaccination records to a US form. Performed by a US Civil Surgeon.) :thumbs:

so many people are confused about this subject and wind up being told little white lies by their local civil surgeons which wind up costing $$$ and not only that in some instances this can be quicker to have completed if your local civil surgeon is booked out until beyond 7/30

also, when you call, it's not technically called an i693a, it's the vaccination supplement to the i693

so many people are confused about this subject and wind up being told little white lies by their local civil surgeons which wind up costing $$$ and not only that in some instances this can be quicker to have completed if your local civil surgeon is booked out until beyond 7/30

Firstly, when you call (and it's not really necessary to call, see below), it's not technically called an i693a, it's the vaccination supplement to the i693

Secondly, the address is Westgate Blvd, not wastgate (just clarifying). It's not always necessary to call, you just need to mail off your DS3025(2035?) along with a cover sheet explaining the need for the supplement to the i693, and the following:

Your full, legal name.

Date of Birth

Passport Number.

Current mailing address &

contact phone number.

Also include a cheque or money order for $10 made out to Dr. LE Arnold.

And that's it! Takes a week or so.

what if i lost my DS3025? I have been loooking for it yeserday and it is just so impossible it is not on my folder anymore, which i knew it was just there. although i still have the appendix C, which also shows that i only need the Td...but can the cs still considers that? huhuhuhu

not sure what the appendix c is, but all you require is the page listing the vaccinations, if u don't have it I fear you may have to go thru the whole medical process again, but I'm no expert

They do exist although it would seem, from the many posts on here, that they are a rare breed. My civil surgeon in Indiana was brilliant and cheap. ;)
