She would use her certificate to get him a US passport. If he has been in her legal custody and a PR also.

As above her son needs to be an lpr in her physical and legal custody. See page 2 of passport form DS11 for instructions on what she needs to submit to get a passport for him. Certificate of citizenship is optional, passport is cheaper and usually faster so suggest doing that first.

He has green card and is in her full legal custody (his father is not in this country). So, to be clear, he doesn't need a naturalization/citizenship certificate to get a passport?

He does not “need” a certificate of citizenship to get a passport or for anything else, though many people like to have the official uscis recognition of citizenship as well as a passport. Many others don’t get one because of the cost, it is personal preference.
