Hi Gang,...getting my AOS info together...I have the typical Vaccination Document I was given after I completed the Medical Exam in Vancouver Canada in Dec 2006. As I have been reading here, it seems this is not enough. This vaccination document has to be transferred to this "I-693 Appendix A" form by a civil surgeon listed in the uscis website.

I have contacted 4 Doctors Offices in Portland Oregon. I have been told they want to do a full exam and

the cost is $300..what the fa......

I explained that I was within the year of the original medical exam and all I needed was the paper work transfer. thats it!!!! They seem to differ with me as I have the written info from the USCIS site.

I really don't want to have to do the exam all over again...to me its a money grab! They refuse

to do just the paper work!

My Question is?...is it mandatory that you have to use a civil surgeon from the state you live in? or could

I hop the state border and use a doctor in Washington state? providing they will just do the paper work for me.

or

...should I just skip this process and just send my original Vaccination Document from

Canada and see what happens?

any suggestions...

It is not manditory to have on in the same state as where you live.

I had similar problem, I did this, visited the doctor with I-485 instructions in hand and pointed to the paragraph where it explains that K-Visas dont need a full medical because it is a part of getting the visa in the first place. Once the doctor saw this he quickly reduced his fee to $35 just the supplement.

Sometimes when you talk to someone on the phone you are talking to a reception person and they don't understand the K-Visas.

Take the I-485 instructions, and also print out this page from USCIS.

And you cannot just send the vaccination doc from Canada, they want a civil surgeon transfer, and will only accept the I-693A in a sealed envelope from the civil surgeon.

Hi Dan!

I mentioned this to the receptionists I was talking to on the phone and they said the Doctors were not willing to

sign any papers without doing the physical exam themselves. I guess its a matter of covering their asses

and making the $300 bucks!

I will keep making more phone calls and see who can give me what I want...LOL

I went thru the Vancouver consulate as well and so did my daughter (K-2).

When I applied for my own AOS, I sent in the Vaccination Worksheet that was given to me from my K-1 medical exam and got approved without any problems.

When I applied for my daughter's AOS, I got the "Vaccination Supplement" done by a civil surgeon in Washington State and they charged us $35.00 to do it. I had to call around to several civil surgeons in the area to find the best price. The doctor's prices varied greatly.

Minor point!

The I-495 only specifies K1 visas. Nothing about K3s.

The link you put does all covers it tho.

When we did the interview [K3/K4] the IO had to research and varify a K3 came under the one year trip also and could use the Supplemental I-693.

Just taking a I-485 instruction print out foe K3s to the CS may not fly . You need a print out of the link you posted.

I called a Doctors office in Vancouver, Washington. The receptionist this time was very helpfull!

She referred me to an office in Vancouver Washington who specializes in INS cases.

360-397-8089 Anna was very helpful. They will do the transcribing for $35 - no exam needed!

Since I live in Portland she referred me to Dr. Torres 503-224-0103. Just spoke with him and he

will do the transcribing for """$10""" Holy #######! I am on my way!

Hope this helps anyone else in the Future.

Its just stupid to pay $300 bucks and do another medical exam!

waited a few minutes while his assistant typed it out and he signed and enclosed in envelope and said to

leave the envelope sealed. I asked for a copy of the I693 - Appendix A form he enclosed and he gave me

photo copy for my records!

Oh yah! charge me $10 bucks ... a far cry from the $300 + and redoing the medical exam as other doctors

offices would ONLY do!

WHEW! another document in place!

I know this message is 3 years old, but thank you, thank you, thank you for the information. Confirmed as of today. Walk-in, no appointment (beware dr torres only works 2 days a week now) and $10 per form for transciption. Everybody else I found was trying to charge me for a full physical and vaccinations, even when I told them I had those done for the visa.

the follow-up vacc's and new physical for step son covered by insurance -- but no civil surgeons on staff at KaiserP to do the UCSIS forms.

Based on timeline data, your I129f may be adjudicated between August 7, 2009 and August 22, 2009.

NOA2: July 23, 2009.

Intervew: Sep. 28, 2009 APPROVED

I arrive BKK Oct 29.

Marriage ceremony (non-legal) 6 a.m. Nov 2, 2552 Hua Hin, Thailand.

Arrive PDX Nov 7.

U.S. Marriage: 20-Dec-2009 Salem, OR

AOS filed 06-FEB-2010 (last possible day for valid postmark)

I live in Florida and had a USCIS doctor in Texas do the transcription with no issues.

My opinion is to get the I-693 or you run a serious chance of getting an RFE. This will add 2-3 weeks to the process and you will still have to complete the I-693 anyway. I wish Dr. Arnold in Texas had not retired, they did our transcription for 30$ and in 1 week.

Keep looking and calling, there are USCIS doctors out there who won't rip us off. We just need to find them........

I would be wary of not doing the transcription in person. The transcription by correspondence has been questioned by some IOs lately.

K-1:

January 28, 2009: NOA1

June 4, 2009: Interview - APPROVED!!!

October 11, 2009: Wedding

AOS:

December 23, 2009: NOA1!

January 22, 2010: Bogus RFE corrected through congressional inquiry "EAD waiting on biometrics only" Read about it here.

March 15, 2010: AOS interview - RFE for I-693 vaccination supplement - CS signed part 6!

March 27, 2010: Green Card recieved

ROC:

March 1, 2012: Mailed ROC package

March 7, 2012: Tracking says "notice left"...after a phone call to post office.
