It doesn't matter what you put because his medical exam report will have an immunization sheet. This is one of those questions that you don't fret over. He gets a visa with either answer. The guy at his interview will not even be interested in this question. This form is used by tourist visas, some short work things, student visas. Not all the questions are tailored to a fiancé who is moving and immigrating. 

 

Warning: You will be really sorry if he waits on his shots. It will cost you money to go do another medical exam just to get some shots signed off in the US. It will be a huge hassle. Get the shots BEFORE the medical exam so he can be signed off for adjustment of status and finished without any further worry about medicals for immigration.  There's only 4 shots for adults. Some he may already have. Varicella can be waived. Most likely he only needs a flu shot and Td (Tetanus/diphtheria). Get shots sorted before everything you're working on now and before making a medical appointment.

Thanks for the answer, He has his doctor appt for Jan 12 - so he can bring the list to his GP you provided earlier then and get whichever shots he needs

Keep in mind that his doctor might say "you don't need that" and it may be true for British medicine. BUT your fiancé has to meet American guidelines which are different. 

An example is the doctor probably won't give a flu shot unless he is a child or elderly. Immigration requires it for everybody 6 months and older. He may have to pay for it at Boots or a grocery store clinic and bring proof of the shot and date to the medical exam.

Thanks for the reminder. Yes, he will bring the list and tell the doctor the situation of why he needs to check what he has had before and what he needs to come to the USA. Hopefully his doctor will give him whatever shots he may not have (if any) from the US list you provided us.

For what he needs from his GP to get for the medical appointment:

-Shot/Immunization records  (Vaccination records)

-Health summary

-GP Summary of any surgeries/previous health conditions

Should my fiance have them send it directly to the Embassy or will they give him hand copy proofs?

Nothing gets mailed to the Embassy.  It is the medical clinic that wants that info from the GP. Take it to the medical exam.
