Follow the link for your answers. Required for all countries where you have lived for more than 6 months since the age of 16 and any country where you have been arrested, even if you were not a resident there.  U.S. police certificates are not required.  Police certificates from certain countries are unavailable or are obtained directly by your Embassy (rarely does your embassy execute this step).  Fingerprints are required for certain countries.

There is no fee for police clearance if you get it directly from  MVS,  https://mvs.gov.ua/ua/pages/Dovidka_pro_nesudimist___katalog_poslug.htm . 

You can order it online,  or make it done in local MVS center, you need passport and Identification code ( for Ukrainian citizen) for foreigners I don't know the requirements .

it take from 8 days to 30 days, and you need "FULL'" " ПОВНА" police clearance, not "SHORT". 

there is no official expiration date on this certificate, but it shows the date when it was done.

For immigration purposes it is typically 1 year
