I will just put my father first and, I hope, that will be adequate. It is misleading to ask fro a family name and a maiden name though...

It's a new form with obvious errors.   They should ask for maiden in both places or not at all

I'm guessing what they are potentially looking for is a close family relationship (1st cousin) which marriage to is allowed in some places but generally not allowed in the USA
