Hey guys, What medical forms will my wife need when she goes for the medical exam before her visa interview at the embassy in bangkok? Would it be the I-683 and one of the DS forms? I think I read somewhere that the hospital would supply the forms but I've also read that they were given the wrong version of the form? Any help would be greatly appreciated.

In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..

You are correct, the necessary forms (and correct versions) are provided by the Hospital.  There is no need to bring copies to the examination.    I can especially confirm this is true for Bumrungrad Hospital in Bangkok. 

 

After the exam is competed to the physician's satisfaction, a sealed package will be provided to the beneficiary which includes the examination results as documented on the necessary forms required by the Embassy.  The sealed package is typically available for pickup from the hospital by the beneficiary 1 to 3 days after completion of the exam.  It is the responsibility of the beneficiary to deliver the sealed package (unopened) to the Embassy during his/her interview appointment.

 

Typically, at time of pickup of the sealed documents from the hospital, an unsealed copy of form DS-3025 should be also provided to the beneficiary along with a CD Rom containing the X-Ray images of the Lung scan.   It is necessary for the beneficiary to keep this copy of the DS-3025 as it documents completion of vacinations and should be provided with Adjustment of Status documentation which occurs after marriage in the US.
