No you cannot. You will get an RFE and if you cant send it on time then you will be deny and will have to reapply. You'd be wasting your money!

This is incorrect. From I-485 instructions, "You are NOT required to submit Form I-693 at the time you file your adjustment application, but may do so if you wish. Because of the time-limited validity of Form I-693, you may choose to submit your Form I-693 after you file your Form I-485. You may also submit Form I-693 in person at an interview in a USCIS field office, if an interview is required. By waiting to submit Form I-693, you may avoid having to repeat the immigration medical examination."

How long have they been doing this? I remember back in 08, you had to send everything in.

I don't know, I just started the process this year. Upon the insistence of my attorney, I included the medical with my package. She insisted that doing so would help minimize delays. I'm not sure if that is true, but my PD is 2/27/19 and I already have my interview scheduled on 5/20/19, so 82 days, which is way faster than I ever anticipated. So maybe submitting the medical with my package did help speed up the process, but it is not required to be submitted initially.

You can apply without the medical exam. I applied in October 2017 without the medical, received a courtesy letter to bring it to the interview or if I receive a RFE asking for the medical to do so at that point. Recently completed my AOS interview (adjusting from a B2 visa)  and presented the medical in a sealed envelope to the IO. Received approval the same day.
