Simple terms - it goes to titles given by royal decree like prince/princess or Sir/Lady that usually bound someone by oath/loyalty. There are many other examples.

I agree, if you are titled a Duke, Earl, Count, etc. based on it passing down through the family then you would answer the question accordingly.
