When using page 12, part 9 to add an additional:

Physical Address: use page number 2, part number 2, but what item number do I list? Physical Address 2 uses 14.a.- 15.b. Item 16 is Your Marital Information. 

 

Employer: use page number 4, part number 2, and which item number?

You put the Page number you are adding additional information for. 

 

If address is on say Page 2 then you put page 2 and list what you are adding.

 

Why would you need to add additional information here. Page 12 is when you need more space. Is that the situation?

Did you run out of room.

Yes. I did not have enough room on page 2 section 2 to list all my addresses for 5 years. Same for page 4 section 2. I didnt have enough space to list all of my employers for 5 years. 

 

Page 12 asks for the Page number, Section Number, and Item number. It’s easy enough to list the page and section numbers. It’s the item numbers that have me hung up a bit.

So I should use the first item numbers listed for that page and section? 

For example physical address 1 on page 2 section 2 uses item numbers 12.a.-13.b. Physical address 2 uses 14.a.-15.b. For physical address 3 (which will be listed on page 12 used for additional information) it should be listed using page 2, section 2, items 12.a.-13.b?
