Name Change on Philippine Passport

Maybe a stupid question, but that is pretty much all I got.

 

When a Filipina changes her name to her husbands name on her passport, you would use the new passport application form rather than the passport renewal form that the Philippine Consulate provides, correct?

 

Going to do the ROM and Passport this week, and I would hate to screw that up.

Yes new passport not renewal.

ADULT RENEWAL APPLICATION

 

Renewal of the following passports will be treated as NEW APPLICATION

  • Brown Passports
  • Green Passports
  • Machine Readable Passports (Maroon)
  • Any older Passports

 

 

  1. CORE REQUIREMENTS
    1.  
    2. Confirmed Online Appointment
    3. Accomplished Application Form
    4. Personal appearance
    5. Current ePassport with photocopy of data page
    6. Original PSA authenticated documents that will support the change of name and/or marital status (if applicable)
      • Marriage Contract
      • Annotated Birth Certificate
      • Annotated Marriage Contract to show annulment/divorce/court ordered instruction
      • Death Certificate of Spouse

Rose did a renewal.  She had a valid passport in her maiden name.  She set up a renewal and got the new passport in 2 weeks.  Her old passport was the maroon one.  She scheduled a renewal but maybe they processed it as new.  All I know is she got her new passport in 2 weeks.

PHILIPPINES ONLY!!!  CFO (Commission on Filipinos Overseas) INFO - Can't leave home without it!

 

PDOS (Pre-Departure Registration and Orientation Seminar) is for ages 20-59.  Peer Counseling is for 13-19 years of age.

It is required to have the visa in their passport for PDOS and Peer Counseling.

 

GCP (Guidance and Counseling Program) is for K-1 Fiancee and IR/CR-1 spouse ONLY.

Honestly, I dont think it will matter. You have to meet in person at DFA and they will know what to do.  The hardest part is just getting the appointment!
