please provide address history for last 5 yrs since my birth I have been living in the same address. so what shall I write there.

 

please provide your last physical address outside of United states of more then one year even if listed above? I am always at the same address so what to write there.

 

Part 3.provide your last occupation outside of United States if not shown above. if you never work outside of United states, provide this information in the space provided in part 7 additional information. till this day I never worked anywhere... what to answer for this question

I have always lived outside of US. Never been to US. since my birth I am in the same address. so what to write ...?

You have to put the date when you "started" living at that address. In your case since you've lived there since birth. Then you have to write that down. I meant, the month, day, year.

but its not asking me since birth just asking me date for last one year of outside of USA.

Yes, it doesn't ask since birth. However, you must complete the form truthfully. Hence, if you didn't put "since birth", you are not providing accurate information.

Not unless you've lived in a different address since birth, then put that address. BUT if not, put your current address.
