Just checking in to say hi! Hope all is well with the fellow VJers. Pete (pjc1973) and I are getting married next Friday. And then the week after we get to start filing for his AOS and EAD!!! Woo hoo!!!

I printed off all the forms and guides to get ready. Any advice or suggestions for making filing faster and more efficient?

What's the I-693a??

I have a really good question too! When filling out forms for the AOS, Affadavit of support and anything else do I (as the petioner) use my maiden name or my new last name? I was going to file my new name with SS and other places after the wedding...or should I file our AOS first (with my maiden name) THEN start doing name change stuff? Is that going to confuse USCIS when they go looking for Moore and start finding Cumberland or would they figure it out?

It's the medical exam for AOS. We were not required to do this nor did we.

I still write my names as FIRST (MAIDEN) LAST NAME to avoid confusion. They know you are required to marry and will change your name. So, it should make a difference either way.

Form I-693A is a supplemental form to form I-693 and shows the intending immigrant's immunizations! It has to be included with the AOS application.

Some people take the chance and not fill in the I-963a form, just submitting the DS-3025 form instead. Some have gotten approved this way, others have had the I-963a ready to go for the interview, and others have had an RFE sent for not having submitted the I-963a originally. Either way, have a great wedding and congrats on making it to the USA!!

K found the 693A and it looks like we just have to submit a copy of his vaccination records he did in process for the K-1 Visa as the 693A itself has to be filled out by a civil surgeon (which was in the UK).

It is advisable to have a civil surgeon transcribe the vaccines he received onto form I-693A.....some people have gotten RFEs for not doing so...others got away with just submitting form DS-3025.

We're getting married 2 days later than you are and are in the same 'AOS preparation mode'. I think we have everything ready except for the I-693A, which we're planning on getting Friday at a nearby civil surgeon's office.

Just a few wee points of clarification.

I693 isn't completed at the consulate level. It is a form used by civil surgeons in the US who perform medical exams for stateside immigration purposes.

I693a is a supplement to I693 - when completed by a US civil surgeon it documents vaccine information.

The title of the the doc who performs the consulate medical exam is 'panel physician'.

US citizens don't petition for adjustment of status. Visa beneficiaries do.

Its worth getting the I-693A form done, it doesn't cost a whole lot and it cuts down your chances of an RFE. Like its been said, some get RFE'd for it, some do not. The vaccine sheet you have from London helps, you take that to the civil surgeon, you can find one near you here, the cost can vary, they put it in a signed , sealed, stamped envelope and you put that with your application.

If you don't want to submit it, thats fine but be prepared for the chance of an RFE for it, I got one done for me just in case, I prefer to lower the chances of RFE's. (However I got one for something else anyway )
