I'm confused. Does he have a CR1 visa? If so, he doesn't need to apply for a green card/adjust status. He just needs to pay the ELIS fee.

If it isn't difficult, it isn't worth it.

9/23/16: Received biometrics appointment letter (day 11)

10/3/16: Biometrics appointment (day 19)

11/4/16: EAD+AP approved (day 53)

HELP. what form do my husband need to fill out for permanent resident card? and what are the requirements? he has a CR1 visa.

You don't have to file any application for green card anymore.

Just have to pay the immigrant fee here, then the green card will be sent to you:

https://www.uscis.gov/file-online/uscis-immigrant-fee

Okay first, you need to READ the visa. It says on it that upon endorsement becomes temporary I-551 valid for 1 year. An I-551 is a green card. So the endorsed visa becomes a green card.

You go here: https://www.uscis.gov/file-online/uscis-immigrant-fee and pay the $165 immigrant fee to produce the hard copy green card.

After your spouse arrives in the USA, their visa is endorsed and becomes a green card. Their immigrant visa package handed over at POE to the CBP is sent to the USCIS. The payment and package are matched up in his file and they produce the hard copy green card. It can take about 6 months. Anything he needs a green card for until he receives the hard copy, he uses the endorsed visa in his passport.
