Any forms after the medical examination?

Dear all,

This morning I called to verify the schedule of the physician where to take my medical appointment and I was surprised to hear that at the end of the visit they do not release any documents, while I thought they would provide you with something (which might be useful at the AOS stage). Do you know if I am actually entitled to receive anything?

At the conclusion of his medical, my fiancee received a copy of his DS-3025 (Vaccination Documentation Worksheet). For AOS, he needed to take this to the Civil Surgeon to prove the only vaccination he needed from her was his flu shot. She wanted to give him all of his vaccinations (over again) so this was proof he didn't need them. She still wanted to administer all of them (more money for her, I guess), but I was adamant that all he needed was the flu shot. I've included a copy of the top of the form which expressly states a copy should be given to the applicant.

Some embassies require the panel physician to send the results of the medical directly to them, others require that the panel physician give the sealed medical packet to the fiancee to bring to the interview. There is nothing else you would need from the medical. If your fiancee passes the medical, the only thing needed when applying for AOS is confirmation that all required vaccinations have been received. If your fiancee doesn't pass the medical, the consular official at the interview should discuss the results with your fiancee and indicate whether there is nothing that can be done or if there are steps needed to overcome whatever the deficiency is.

I received a copy of the vaccination worksheet (only after specifically asking for it) and a copy of my x-ray on CD-ROM. That was in 2012, however, so I do not know if anything has changed. I had my medical at the Frankfurt physician.
