If after getting married, my spouse decides to keep her maiden name, without including mine, as her last name, could that hurt any chances of getting a visa and eventually a green card?

I will be filling out the I-130 and the other forms required to get the CR1 visa.

Not a problem at all. It is not a legal requirement for a woman to change her name upon marriage, it is merely an option open to her.

If she wants to keep her name, it will have no effect whatsoever on her visa. It's her choice and nobody else's business.
