Hi everyone,

I am filling out the petition and the biographical data sheet and am a little confused as to which name I should put. I was born with a different last name than the one I am using now. I went to court to change my name back in 2011 because my biological father has not been in the picture since I was born. Should I put my new name and provide court documents or my old name since I'm not sure if they will look up my SS number and see the old name. Also, should I put my biological father's name on the 325-A or my guardian (real father) who has been in my life for my entire life? Again, I'm afraid they'll look up the number and see my biological father's name instead.

There is space on the form to list other names used. That is where you enter the name you used prior to 2011 (along with any other names).

When listing your father, this is birth father (what shows on the birth certificate).

Submitting a copy of the name change paperwork wouldn't hurt.

You want the information you submit to match up with the background check.

Totally fine to do that, need to submit copies of every page, even the blank pages.

Use your current name and list your birth name under other names used.

You should put your biological father's name.

You should submit a copy of the name change.
