Parent’s information in G-325A

My father died 3 years ago. What's the right way to put it into form G-325A? Do I still have to present his Name, date and place of birth and place where he died?

For mother, do I have to type existing last name and maiden name or only maiden name?

My fiance's father died few years ago as well, so i did write his birthday and location where he was born but where it's asked where does he live now i wrote DECEASED

I had the same question about mom's maiden name and was told to write ONLY maiden name.

I wrote "deceased" for my parents as well.

Maiden name is what they are looking for.

If they are looking for you mother's potential "married" name then they have that information based on your father's information (traditionally speaking). I am not exactly sure what they do with this information but just give them what they ask for and try not to read more into the question....

I agree with all that has been said. Deceased for the father and just the maiden name. That is how I did for my fiancé's G-325A. Good luck!
