So it appears in our AOS we missed something but I don't really quite understand what they want. It says to submit a propertly completed Form I-693 and vaccination record. Then in bold it says The medical examination section does not need to be completed, but vaccination record needs to be completed by civil surgeon. I have the vaccination document from St Lukes in Philippines which I thought we sent them but perhaps I'm wrong. Has anyone else had something similar? I can't understand why they would be requesting information that they know was performed before she came to the US?

The vaccines she got were MMR, Tdap and Vericella which I think is all that is needed.

She still has to have a Civil Surgeon in the USA sign off on the vaccination record,somewhere in the paperwork you have should be a list of civil surgeons in your area. Pick the closet one and get him or her to sign off on the form. That's what they are looking for on the form is that he or she signed off on it. And I am sure some fees will be involved.

You did not list the flu shot. If the exam was during flu season, Oct. - Mar., then a flu shot is required.

You just need to attach a copy of the vaccination record to show that your vaccination has been done. There is no need to get it certified by anyone, just a copy of the record will do.
