Can anyone explain the i693a for us:

- who provides the form? us? or the physician/medical practice?

- will the physician/medical practice need to see the original vaccination sheet (currently in our case file), or is a photocopy enough?

- is $120 too much to pay for the varicella vaccination? that's the cheapest we've been quoted (by anyone who sounds like they know what they're doing)

the civil surgeon/physician will have this document.

keep phoning around different medical doctors, you do not need to have the vacc done at any particular doctor just as long as you take evidence of having it to the CS

The Civil Surgeon has the forms and fill them out for you.

The CDC has a list of places that gives vaccinations for free. Just make sure you get proof of the vaccination from the, to show to the Civil Surgeon. I'll try to get the link.

Or you can continue to shop around until you find a CS with reasonable rates.

Get the vaccination done at a local heath clinc.... and just take proof that its been done to the Civil Surgeon.... I paid $15 for 6 shots and the polo drops...

the ds-3025 that you should have received from the medical is the form you take to the nearest civil surgeon listed on the uscis link above. you must have the i-693a transcribed by a uscis approved civil surgeon. such cost me $40 for them to transcribe this onto an i-693a. you should get a sealed envelope to submit with your aos package and a duplicate of the i-693a should you need it for later.

We just went thru this today. The problem is, you don't know what shots your SO will need until you visit the civil surgeon. We showed up today at a local civil surgeon on the list, and he said she needed the chicken pox shot, which is the same you stated. He said I could go and get it at my family doctor or other places as others have stated. I just had him give do the shot there instead of making arrangements and waiting for an appointment at one of those palces and then returning to the civil surgeon. I paid a total fee for the shot, appointment, and the I-693A form of $120.

Well, I envy those of you who manage(d) to get the varicella vaccination plus I-693a form for $35, $20, or for free.

We’ve had no such luck, despite living in a major town with a comprehensive array of medical services and a long USCIS list of local civil surgeons. The irony! – we even live just one block from the local ‘hospital district’!

In our town and the ten-kilometre radius around it, after calling around 30 clinics or physicians, the cheapest price we’ve found for the shot and the form is $124. Even the local health dept charges $65 just for the shot (no I-693a).

Guess we just have to stick it on the ever-growing immigration tab...
