We never received a copy of the DS-3025 at POE. She only has an X-ray and yellow vaccination card listing tetanus, Hep B and some other indecipherable vaccination. So if we go the Civil Surgeon, she's going to have to complete an entirely new physical and set of vaccinations & find an affordable CS in the Philadelphia area (Dr. Emil P Sfedu is the guy that comes up on VJ as affordable). Based on the research I've done on VJ & the following links, I've decided to include a written statement in place of the I-693 stating:

Please note that I am adjusting status from a K1 visa. I had a medical exam on [Date] at Holy Trinity Medical Center in Accra, Ghana. Because 12 months have not passed since that exam, I am not required to have another medical exam. Please see my medical files which I turned over at POE which include the original DS-3025 showing all completed vaccination requirements.

So she's going to file without the DS-3025 or visiting a Civil Surgeon to complete the I-693, not knowing whether or not the DS-3025 was filled out correctly, and keep our fingers crossed and hope she doesn't get an RFE or NOID. If she gets an RFE, she'll visit the civil surgeon and have the I-693 completed.

The question I have is regarding the RFE, and NOID's people are receiving for incomplete DS-3025's. Will I have enough time to schedule an appointment with a civil surgeon and get the blood results in time to respond to USCIS in 30 days?

I'm also reading that local offices are sending out NOID's to K1's who don't submit evidence of shared life with the original I-485? So should I be sending wedding pics, joint accounts, medical insurance etc. with my AOS petition too?

  • They don't give DS-3025 at PoE. The beneficiary got one from the clinic after medical exam.
  • Many people don't have many evidence shortly after they got married from K-1. My husband just enrolled me to health insurance TRICARE and other benefits after we sent AoS package. You can provide those for interview but the evidence that you have mentioned such co-mingling is not required for adjust of status from K-1 visa.

It is honestly a hit and miss situation. If you include the letter I would edit the letter and copy this down. USCIS knows you had your interview within the past year as they have a copy of your DS-3025. So this is what I would write down:

Unfortunately she never received any medical information besides the yellow vaccination card for the passport & x-ray results

I had my medicals from Akai House and was not given a copy of DS-3025 either.

However, I had a card with all the required vaccinations I received after the medicals in Ghana. I took this to a civil surgeon here to simply transcribe onto form I-693 ( cost me $40).

There was no need to complete a new medical examination because, I filed AOS within 1year of my medical exam in Ghana.

05/27/14 - Sent AOS package via USPS05/28/14 - Package received by USCIS06/02/14 - Text and Email NOA1s received06/06/14 - Hard copies of NOA1s received06/10/14 - Biometrics appointment received (for June 24th)06/24/14 - Biometrics completed08/06/14 - SMS/Email notifications of EAD/AP combo card in production08/11/14 - EAP/AP card received.<p>09/13/14 - "Notice of Potential Interview Waiver" received04/13/15 - AOS Approved

So she should be good with the yellow ministry of health card she received? I just scheduled an appointment for Saturday with the Civil Surgeon I mentioned above and the price they gave me was $48 so hopefully that card is what they need. Thanks for the feedback too.

I didn't receive a DS-3025 either, Since I also came here on a K1, and had a medical in December, I didn't include anything, not even a letter. Though if I had seen one on here before I filed and thought of it I probably would. So far I have not had an RFE..and hoping I won't!

Yes she should. Just ask them to transcribe the information on form I-693, and please remember to ask for a copy. I got a copy at no cost just for my personal records.

Well I didn't go to a civil surgeon. I just used the vaccination card I got from Akai House. I know there was a copy inside the packet they gave me at the Ghana consulate to submit to immigration. At the point of entry the immigration officer told me they will keep it for further processing. I also didn't add pictures and evidence of co-mingling when I was adjusting status. So far I haven't received and RFE

My husband also went to Holy Trinity and didn't receive any documentation aside from the yellow vaccination card and his chest x-rays, we have been trying to figure out our next steps for the AOS... It seems like the common route is to contact the CS and have them transcribe that which is on the vaccination card to the i-693? There are two CS in our area, so I guess we should get calling and see who has the better rate?

He went for his medical in May, so it hasn't even been 6 months.. this piece of the AOS process is more confusing than anything we had to do for the I-129F..
