I made a checklist for my AOS (Adjustment of Status), EAD (Employment Authorization Document) and AP (Advanced Parole) paperwork. Maybe it can help some of you out as well. ;)


I am adjusting from a K1, but I think the forms needed are pretty much the same for all family based adjustments.



Form I-485 Packet and Supporting Documents (AOS)



-Form I-485 Adjustment of Status


-Personal check in the amount of $1070.00 made payable to U.S. Department of Homeland Security


-G-1145 Form for E-notification


-Copy of I-797 - Notice of Action, Approval of K1 petition


-Medical Vaccination Form DS-3025


-Medical Results in sealed envelope from Civil Surgeon


-G-325A form, Biographic Information


-2 Passport Size Photos


-Copy and translation of Birth Certificate


-Copy of Passport Biographical Page


-Copy of K1 Visa Page and entry stamp


-Copy of Form I-94


-Copy of Marriage Certificate


-Form I-864, Affidavit of Support


-Copy of Federal income tax return, Form 1040, for 2011, 2012 and 2013



Form I-765 Packet and Supporting Documents (EAD)



-I-765 Employment Authorization


-G-1145 Form for E-notification

08/01/12-Married08/17/12-Applied for Social Security Card09/23/12-Husband received his Social Security card!09/27/12-Filed AOS09/28/12-AOS package received10/01/12-Text message from USCIS10/03/12-Check cashed10/09/12-NOA1 for I485,I765,I131 AND Biometrics Appointment Letter10/24/12-Biometrics Appointment12/11/12-EAD and AP Approved-75 days12/14/12-EAD/AP Card Production ordered12/21/12-EAD/AP Card came in the mail06/22/13-Green Card Approved06/27/13-Greed Card Production Ordered06/29/13-Green Card came in the mail<p>

04/09/15-Mailed ROC

04/10/2015-Package received

04/14/2015-Check cashed

06/02/2015-Called Service Center (still no NOA1) Service request found out wrong address on paperwork! :(

06/03/2015-Called to get address updated Ar11 online add change didnt work-got infopass for June 10th

06/10/2015-Went to info pass appt. Hubby got a 1yr. Extension stamp in his passport and biometrics done

08/12/2015-ROC Approved. Got letter in the mail.

08/29/2015-received new card in the mail.

From the I-864 instructions

If you provide a photocopy of your tax return(s), you must include a copy of each and every Form W-2 and Form 1099

that relates to your return(s). Do not include copies of these Forms if you provide an IRS transcript of your return(s)

I have heard of people getting an RFE for not sending in photos for each application, so I thought I rather go the save route and send them photos for each one. ;) But it's good to hear that it worked out for you!

I posted the duplicated in case of filling separately. But thank you for the input! ;)

How about the rest of the supporting evidence for i864? like w2's for those 3 years, paystubs and employment letter?

Do you really need the w2's for all 3 years? On the I-864 they are only requesting the most recent year of tax returns, the other two years are voluntarily. I didn't know that you HAVE to send in w2's and pay stubs, I thought just the tax returns are sufficient, but thank you for letting me know!! :)

yes, if you send 3 years of tax returns, then you need to send w2's for those 3 years as well. I suggest you (re-)read the instructions again, especially page 5 and 6

yes, if you send 3 years of tax returns, then you need to send w2's for those 3 years as well. I suggest you (re-)read the instructions again, especially page 5 and 6

I did read the instructions, and I now know why it's not included in my list. My fiancé is retired so there are no W2 (only 1099's), but it was my bad for not including them for other people that do work.

Then you need the IRS transcripts or a complete copy of his returns with all forms, schedules and 1099s.
