I'm not sure how to read my wife's DS-3025. Is she supposed to get a Td and Varicella vaccine or not because it says that applicant may be eligible for waiver? If she needs vaccines can she get them done by her doctor under our insurance and then have a panel physician see that and fill out the form? I'm just not sure and I haven't seen a clear answer on this forum yet. Thanks in advance for any help. I've attached a redacted copy of her DS-3025.

If anyone sees this thread in the future the answer was that she does not need any more vaccines. We are good to go to file for AOS.

You're right, she doesn't need more vaccines. However, it looks like nothing was checked in the "rotavirus" row which seems to be a common isue for people going through St Lukes. You might want to get it transcribed to the I-1693 and have the civil surgeon mark off "not age appropriate" as rotavirus is only given to babies.

With the rotavirus left blank, there is a chance you will receive an RFE and need to see a civil surgeon. You can either go ahead and do the I-693, or worry about that if you happen to get an RFE for it. There was someone else recently that went through Manila and were approved even though the rotavirus was left blank. There have been those that have not been as lucky though.

so i am in the same situation, the embassy neither border officer did not give back to me my medical examination record

so i guess i am applying for AOS without medical results.
