Check the USCIS website/....they list them for every area.

Call on and tell them that you only want the vac supplement.

That's where I got the phone numbers for the ones I have already called. All the ones I have called are telling me they won't do just the supplement.

Welcome to our world Merle,

I am having the same problem in Miami.I am now able to find ones that will do just the supplement buuuut they all want $ 75.00 US or more!!!!I really belive that this is a SCAM.If the USCIS had the time to investigate these Doc's(scam artists) maybe it would not be such a LARGE problem.Oh and a way that I was able to find many other doc's in the South Florida area was to get out the phone book to the Zip code page .I just started using the zip's as they got further away from us to use as the look up in the USCIS site.

GOOD LUCK We still do not have the supplement yet.If push comes to shove I will pay$$$$$$$$$$$This SUCKS.Oh here is a copy of the supplement if it will help.

Hi (F)

There used to be an old thread but I can't seem to link to it so here is the info...

Dr. Kevin O'Keeffe, OCCUHEALTH

726 Broadway, Suite 201

Seattle, WA 98122

Tel: (206)682-2234

I spoke with Denise one of the receptionists. She was fantastic but I am sure all of them are good! She let me do it all through phone/fax/mail. I spoke with her on the phone and she knew exactly what I was talking about. I faxed her over the information for my husband and the vaccination records. Once she verified that he had all the shots he needed on his vaccine form she had me mail a money order for $35. After they recieved the payment they filled out the form and mailed me the original in a sealed envelope and a copy for my records...overall it cost about $40 for mailing, the fee, and the money order. I HIGHLY recommend them. If you are in the area I am sure you would get the same service in person.

Good luck and let me know if you need anything else.
