Want to provide the checklist and timeline for applying a US Passport for an infant since we recently went thro' this with our kid.

Documents Needed:

  1. Completed passport application - you can fill this online or by hand. If you do by hand, make sure to use a black pen
  2. Evidence of child's US citizenship - we submitted original birth certificate, this will be returned separately after passport processing.
  3. Evidence of child's relationship - the original birth certificate had both the parents names.
  4. Child's recent color photograph - we took several passport sized photos with our camera at home and ordered prints at Walmart.
  5. Parent's Identification - we carried our US Driver's Licenses.

Fees: $80(cash or check) made payable to Department of State and $25 for post office(this can be paid with credit card)

The process:

You can goto any acceptance facility but we decided to go to a post office. Make sure to call post offices around so you have an appointment for applying US passport. Usually they are booked up for several weeks. We called few and some were booked for 2-3 months but we ended up driving about 30 miles to the nearest one that was available.

Both parents have to be present while applying for an infant's passport otherwise if only one parent appears the other one must submit a notarized written statement.

Our appointment was at 11am we showed up a little early. The postal clerk called out our name, verified our documents, took photo copies of our Driver's License and made sure that my spouse's name was corrected(has to be a maiden name not the current name).

We took 3-4 different passport size photos and the postal clerk accepted only one of them letting us know that the others did not conform to the photo requirements(some had shots too close up etc) Otherwise it's $15 to take a photo at the post office.

The postal clerk then had us take an oath and then collected the fees and processed the application.

Timeline:

Nov 22, 2013: Application Submission at Post Office.

Nov 27, 2013: The $80 check was cashed.

Dec 10, 2013: Passport was mailed from Little Rock, AR.

Dec 14, 2013: Passport was received.

Dec XX, 2013: Still waiting on the original birth certificate.

Total time to receive the passport, 3 weeks and 1 day.

We were unable to track our application online and we registered for updates as well but glad to have received the passport in under a month.

06/15/2010 : I-130 for son
06/23/2010 : Check Cashed
06/26/2010 : Received NOA
03/20/2012 : Received RFE(I-797E)
05/23/2012: Application approved after RFE response
06/29/2012: Received letter from NVC

06/03/2015: Change petition from F1 to F3 category

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).
