No, it won't be a problem as far as the medical exam and interview are concerned.

I think what you need to be concerned about are the amount of special child care and education costs in the US.

Why should I be concerned abt that ? I love my fiance and her children we have known each other for over 2 years had one baby together that died very sad to say. I have a good job and ins . there are no other options

The reason you should be concerned is that while DS is not an issue for the medical or interview, the extra financial burden that his condition places on you will be considered for the visa. Ie if you were just scraping past the poverty line, they may deny you, arguing that you need more money to look after his needs (special education, medical issues such as heart conditions common to those with DS etc). I recommend bringing proof of insurance with you to the interview.

Me: Irish/ Swiss citizen, and now naturalised US citizen. Husband: USC; twin babies born Feb 08 in Ireland and a daughter in Feb 2010 in Arkansas who are all joint Irish/ USC. Did DCF (IR1) in 6 weeks via the Dublin, Ireland embassy and now living in Arkansas.

made over 60k last year. not rich but more than scrapping by

Yeah with that income and if you can show proof the child will be covered by your insurance, you should have no trouble.
