Double check that since only the person being sponsored is the spouse, that only Part 3, #1 is checked as Yes, that there are no other marks/writing in any of the boxes in 2-6f, and that #7 says "1".

If all of those were already done, call and check to see what they say. Otherwise, just resend it.

Was it signed in black ink?

Did you enter any names into part 3 or just check off Part 3, Q1 as Yes?

The rest should be blank if you are only sponsoring the beneficiary and no one else.

I think so. They are new checklists to go with the new I-864 I assume. I suggest you submit the whole thing over again with the page he messed up on as corrected, making sure he signs in black ink, and using the bar coded cover sheet they sent you. To make sure that he prints it out properly he should set the printer to black and white or greyscale only.

Oh you probably do not need to send the supporting evidence again, just the I-864. They will add it to your case.

yes, new signed , dated, and completed i-864 form (complete all questions even the correct Q with the change noted in the cover letter) + barcoded cover sheet and the cover letter they have sent biggrin.png

You don't need to send all the supporting documents again. Just send the new form with corrected item and of course with signature. We received a checklist too but it's part 5 on item no. 8. And I put our case number on the upper right hand on all documents we are sending them just in case they will misplaced one of the pages. :)
