HELP!!! Can't find a Dr. to Transcribe!!!

Hola, I work in a Health Department in Mississippi and we transcribe vaccines. Try ur health dept they should be free. we do them here.

You must use a designated civil surgeon to transcribe the vaccinations onto the I-693. You cannot just get them done anywhere by any doctor. You can get any missing vaccinations from your own doctor or the health department, but then you must take a record of receiving those vaccinations to a civil surgeon to have them transcribed onto the I-693.

Hmmmm..... So we went today and all I got was a large brown envelope that is sealed with her name & date of birth on the top right hand corner. No tape, stamp, etc. They said this is how they do all their I-693's. Is this acceptable?

Also the other vaccinations don't have anything listed. Is that important or not? I'm showing you a picture of the copy of the form they gave us.

The form itself looks a little less filled out than ours, but I would say okay?

Except... I'm not too convinced about the envelope not being sealed more. :unsure:

Anybody could just easily open the envelope and mess with it, which is what USCIS will probably think, too?

Maybe someone can give their two cents. :blush:

(* I was going to provide pictures myself, and then I realized we have already given our envelope to our advisor for filing. Sorry! :bonk: )

I did an I-693 for my ex-wife back in 2003, she was an immigrant as well. We were given a letter sized envelope with a squiggly stamp on the rear of the envelope where the seal was.

Come to think of it though, our medical in Mexico only came in a black plastic envelope that would technically be easier to open than this, so I guess its all right.... even our packet we had to get processed at the border was just a brown envelope like this, so I guess it is all right come to think of it.

Our was with tape and office stamps across it. The 3 box was checked for us in I-693, stating all vaccines completed and met

Not sure why he did not transcribe all the vaccinations if she provided records of them. He has marked all the ones required for AOS, so should be ok. I think he checked blanket waiver because he marked that all the shots in the in the series were not done yet, insufficient time interval, too soon to get the next one. VH is what they mark when you had the chicken pox already, varicella(virus) history. Only the first in a series is required for AOS.

I would not worry about how it is sealed in the envelope.
