The family name can be her maiden name or her married name, it is your decision what to use.

Page 5 of the I-485 instructions on USCIS's website answers your second question, http://www.uscis.gov/files/form/i-485instr.pdf, you are setting yourself up for disaster if you do not take the time to read the instructions for each form you are going to use.

If she will be legally changing her name to yours after you marry, then you use her married name on all the AOS forms. The name you use is the name her EAD/AP and green card will come in.

Everyone mails their AOS paperwork to the same address. It is listed in the I-485 instructions.

If she will change her name to your name then her MARRIED (your) family name goes in the "family name" space and her maiden name goes in the "other names used" space.

Check the example forms here at VJ The form instructions tell you what address to send it to.

Ron, is Sveta taking the name of 'Moore' ?

If so, that's the name to use on the I-485, for her.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !
Ya know, you can find the answer to your question with the advanced search tool, when using a PC? Ditch the handphone, come back later on a PC, and try again.
