Hi,

I'm filling form I-765 for employment authorization and question 11 ask if I ever applied for employment authorization from USCIS and I don't know what to enter in that field.

I had two CPT (Curricular Practical Training) during college which required the university to notify them and is listed on my I-20 but according to the university that doesn't really require their authorization as the OPT does. They do not issue an EAD card for that they just list that under my I-20. Should I answer yes or no?

I also didn't understand what is my category to fill the field 16... Anyone that was on F1 and got married knows what I have to do?

Also the $380 fee is not required for change of status right? I am on F1 visa right now.

Another general question:

Can I staple the forms ??

Should I put N/A in every single field that I don't really have any information to fill such as middle name, Apt #, A# etc or can I leave it blank?

Question 11.. really simple, if this is your first time applying you answer NO as you never applied before.

Your visa is the visa you got to the US... If you got to the US on a F1 then that is your visa... If you got to the US via a petition from your fiance then that would be a K1 visa.

No form stapling..

Yes, put N/A where not applicable this tell them you answered the form and not just ignored it.

No on field 16 doesn't ask for my visa...is asking for eligibility category and I have no idea which one is mine:

16. Go to Part 2of the Instruction, Eligibility Categories. In the space below place the letter and number of the category you selected from the instructions (For example (a)(8), ©(17)(iii)).

I have no idea what is my category I think should be C9 but I didn't understand what I have to put in the three parenthesis that shows over there. Anyone changing status from F-1 after getting married knows how to fill that?

It's free when filed based on code C9.

They want to know about previous EADs, so if you didn't have one for the CPT, then no.

- 7/23/2008 came to US with F-1 visa
- 9/11/2012 I-485, I-130 and I-765 package sent to USCIS Chicago lockbox
- 9/12/2012 package delivered and signed by XXX
- 9/14/2012 received email with receipt numbers
- 9/17/2012 check cashed
- 9/30/2012 received hard copy of biometrics appointment on 10/19/2012, notice dated on 9/21/2012
- 10/12/2012 successfully walk-in one week earlier without asking reason at New York Long Island City office
- 11/5/2012 started to work in a new firm
- 11/11/2012 EAD approved!!! AWESOME!
- 11/19/2012 EAD card delivered, one day ahead of my birthday!
- 1/11/2013 received txt and email about interview time
- 2/14/2013 interview - Got APPROVED on the spot!!
- 2/27/2013 USCIS online update - 485 decision!! we mailed you a notice that we had registered this customer's new permanent resident status :) Later, my 130 update too!!!
- 3/4/2013 Card Production!!

Do you know how I have to fill the 3 parenthesis?? I saw on the example here they filled (C ) (9) ().

Should I leave the last one blank?

C-9-blank or blank-C-9 should not make any difference.

My lawyer filled mine in as C-9-blank and I received my EAD without issue.
