Hello guys! it's been 4months since i got here. I just receive a letter for interview this coming October. I'm confused if i should comply the form I-693? because they say;<br class="Apple-interchange-newline">I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form

Now it says here in my vaccination worksheet the result; VACCINE HISTORY INCOMPLETE and APPLICANT MAY BE ELIGIBLE FOR BLANKET WAIVER BECAUSE VACCINATION NOT MEDICALLY APPROPRIATE.

Should i go see a Civil Surgeon before my interview or NOT?

Any suggestions?

Thank you for sharing the link! i think i have the same worksheet that you mentioned. Actually there is no missing vaccines, they ticked "not age appropriate" the rest of my immunization. They only gave me 3 vaccines namely Dtap, MMR, Varicella with dates given.

So is does this mean I'm good to go for my interview right?

Yes, the worksheet you're talking about is the Vaccination Record, or DS- 3025 (look at the bottom of it). If your medical was done less than 1 year ago, all your vaccines are done, and the rest is ticked "not age appropriate" (that was my case also), you don't need to go see a civil surgeon. But you still need to show proof that your vaccination record is complete, so have you DS-3025 with you (and make a copy).

Did you submitted a copy of it in your initial AOS package?
