5 months usually refers to the time between the filing of the petition and the approval date of said petition. It can be less or it can be over 5 months; but 5mo are considered standard processing times.

5 Months from the date they receive your application (upon which they send you the NoA1) until it is approved by USCIS (upon which they send you the NoA2).

After that the application still has to be forwarded to NVC and then to the consulate after, and then your fiance has to follow the procedures of THAT consulate as far as how to receive his package, what documents to acquire, how to send in his filled out forms, etc etc... and THEN there's the interview.

In our case, it was slightly less than 5 months from the date I mailed the petition to the approval of the visa in Mexico. Seemed like a long time then, but thinking back it went very fast!
