Need help for AOS

hello my VJ friends! i'm preparing my documents for AOS,hopefully we will send it next week.

i'm confused with the forms.btw,i come here on K1 visa.

1.)Form I-693(Report of medical exam. and Vaccination record)

do i had to put my name on what is in my passport?.we got married last june 15.and i had my

marriage cert. with me now...

2.)I-765(Application for Employment Authorization)on this form in #16 say's go to part 2,

what should i put in there?

3.)I-131(Application for Travel Document)

should i fill up this form in my name of what is in my passport?.I want to used my married

name on my Employment Authorization and Advance Parole and GC.

4.)I-485(Application to register Permanent Residence or Adjust Status)in this form i want to

fill up using my married name.and attached my married cert.

5.)I just went to Social Security office yesterday,for my SSN they said i will received it

after 2 weeks,should i wait for it?is it really required to put your SSN in all this form?

because all this form ask for SSN...and we really want

to get this thing done next week.

really need your advice,specially for those member who already went through this.

I-131- if you want to add your husband's last name JUST PUT IN ALL formS your married name

I-485- Add your husband's last name if want to use his last name.

If you don't have SSN yet just put NONE

I-693 - You are not required to have another medical examination as long as your Form I-485 is filed within one year of your medical examination, then you can just send a copy of the Vaccination sheet (DS-3025)

I-485- Add your husband's last name if want to use his last name.

If you don't have SSN yet just put NONE

I-693 - You are not required to have another medical examination as long as your Form I-485 is filed within one year of your medical examination, then you can just send a copy of the Vaccination sheet (DS-3025)

But for the medical you must make sure that your medical shows all vaccines were given and not waivered, otherwise you might have a problem and need a vaccine transcription.

The I-693 is to be completed by a CIVIL SURGEON, they will transcribe the vaccination record (3025 I believe it is, anyway something like that), then they will seal it in an envelope and give you a copy... that envelope is what you send with the application.

My wife used her married name for ALL documents as it asks for her LEGAL NAME, and her married name IS her legal name. (And her SSI card was issued in her married name also)

As for the # 16 question, if you did not check YES on question # 11 you do not complete beyond there.

No you do not need to put the SSN on the forms, they state on some forms "if available" for the SSN and the A number. BTW the A number is on the NOA2

... and yes my wife's middle name is now changed to her "old" maiden name on ALL documents, including her SSI card.

hello there! thank you so much for all the information...i just got my SSN yesterday they put my maiden name on it not my married name...how many passport photo you send for this AOS?

6, 2 for each form. Follow the instructions for each form and provide the documents it asks for on each separate one. They are separate applications, but are all mailed in together.
