No, the I-485 only requires a marriage certificate for K-1 entrants for proof of marriage.

Not with the application. If you get an interview they will expect you to bring evidence then.

She gets 90 days from when she enters the US. She gets 6 months (usually) to enter the US.

She will be given an I-94 upon her arrival. The K-1 visa will become null and void once used to enter the US. The I-94 will be good for 90 days, and it is what controls her status here in the US.
