Alright, everythings really to go to the Dallas Lockbox...just wondering if I'm paying the correct fee? $340? Finding conflicting info on govt sites. Thanks.

And the proper way to make out my personal check is...? (just double checking! cant ever be too sure!)

EXACTLY like the instructions tell you. Make payable to: "US Department of Homeland Security" unless you live in Guam or the Virgin Islands or outside the country.
