Is this required? Should I include this in the application package? Never heard of anybody need this letter...

It is not a requirment of the application and is not included in the instruction letter I have for the interview, but they can require whatever they want. Wouldn;t take but a minute to whip one out and sign it.

Notary is not required as a notary does not testify to the matter of the letter, only that a particular person signed it. Was the spouse at the interview?

It's not required. I've only heard of it being asked for a couple of times, when the married couple had little proof of living together (no joint bills, bank accounts, kids etc).

Would have been more than delighted to go in with my wife, Swear to God on a Bible we were living together, and sign it rather than spending hours and hours digging out evidence and copying over an inch thick of documents.

Or they could be like these dog rescue places that come to your home and examine it to make sure its a nice place for one of their mutts. Question her daughter living with us, our grandkids, neighbors, our dog, see our master suite with a king size bed, shared bathroom, pictures of us all over the place, co-workers, anything they wanted to do, we have nothing to hide.

But instead, her IO wouldn't even look at me, just wanted to see are papers. Crazy.

That's probably just in case some other IO has to handle the case and will need to see papers too. You cannot keep going there and swearing on the bible whenever they need to know something, they just need to have valid proof that others can read and see for themselves without having to explain anything or to request more evidence whenever it may be needed :)
