Hi

I'm working on my AOS (K1 just married) package and have a question about the medical element of it:

I have the DS-3025 form and plan to send a copy (They took my medical envelope off me at immigration). The results section: the first 2 boxes are checked (Vaccine history incomplete and applicant may be eligible for blanket waiver because vaccination not medically appropriate)

At the medical I was told I was missing the 2nd MMR shot - I was told I could have that at my own doctor since it was only needed to be done for AOS. So on the form against mumps I have 'K1 visa applicant has chosen to defer vaccine at present time'. I got the 2nd MMR shot at my own doctor and got a print out of my vaccine record to show this before I left the UK.

My question is: Do I send a copy of the DS-3025 with the vaccine record from my doctor attached, highlighting the 2nd Measles Mumps Rubella shot record? Do they need to know I had the second shot or do they just care about the first which I had as a child and is listed on the DS-3025? I don't want to include the extra info if i don't need to, confuse them and cause an RFE to go do the civil surgeon form.

You do know that is the medical was incomplete in the UK that you need a Civil Surgeon to bless off on your paperwork since you arrived here, right?

They don't need to see the second shot for AOS. You only need to show that you have started the process. Some shots were not available in the UK so my fiance had the appropriate box checked, but then when she arrived in the US and the shot is readily available, she received it. Some shots are also very expensive in the UK and cheap in the US so we decided to delay those until she was here. I sent EVERYTHING medical that I had. What really counts is what the Civil Surgeon is going to write on their form. They are the ones to say that you are current or that you are missing something in rhe shot department. Then they seal the envelope so you cannot mess with it.

03-23-2011 - NOA1 received in mail, dated 03-09-2011.

05-31-2011 - RFE requested. They want better passport pictures of me.

06-06-2011 - Additional passport pics sent.

06-08-2011 - Evidence received and acknowledged. Whew!

06-16-2011 - NOA2 received!

07-20-2011 - Packet 3 Received!

08-01-2011 - Packet 3 returned to Embassy.

08-22-2011 - Packet 4 Received!

09-19-2011 - Interview...APPROVED!

09-23-2011 - Visa in Hand

09-29-2011 - POE LAX

11-11-2011 - Wedding at 11:11pm GMT time.

AOS Journey

12-02-2011 - Mailed in AOS/EAD/AP paperwork.

12-05-2011 - Delivery confirmation per USPS.

12-27-2011 - (3) NOA I-797C received, dated 12-20-2011. Biometrics appt set.

01-10-2012 - Biometrics.

01-20-2012 - Notified of interview appointment for 2-21-2012.

01-31-2012 - EAD and AP approved.

02-08-2012 - EAD/AP card received.

02-21-2012 - AOS interview approved. EAD/AP card confiscated.

03-01-2012 - Green Card in hand!!!

364 days total time!

No.

You take your DS 3025 AND your vaccination record to a Civil surgeon and they complete the vaccination portion of the I-693, then you send the I-693 with your adjustment.

The only way you can use the DS 3025 by itself is IF the vaccination record is complete before you arrive and is so noted on the DS 3025.

Google "Civil Surgeons (your state)" and start calling for appointments. They will tell you that you need a full medical exam...you DO NOT, you had on within a year at an overseas panel physician. However the CS can still require one in order to sign off on your form. They basically can do anything and charge anything they want...welcome to the next level of immigration parasites...civil surgeons. First you get the unnecessary lawyers, now you get the unnecessary doctors. Prepare to pony up as they say. Call around and get prices.

Additionally, what they tell you on the phone and in person can differ! When I called around, one surgeon said that they could just give me the shots I needed and sign off the I-693 without doing a full medical. When I actually GOT to the office, they told me I'd have to have a new medical. I took the instructions with me regarding the I-693 and had highlighted the part about not requiring a new medical (just in case), and they were then willing to just give me the shots I needed without completing a whole new medical as well, so maybe take that information too as back up? I sometimes wonder if they try and charge as much as they can if people don't challenge them about it. Apparently noone had just had the shots before for the I-693 (which I find hard to believe - this was Ohio State Uni Campus in Columbus - not like they wouldn't have seen that before!)

As everyone else said you elected to miss the shot at your K1 and therefore need to see a CS here in the US. Had you had the shot so that you were up to date at the time of that medical you COULD have just sent the DS-3025.
