Does anyone know if the bold box at the bottom of this form that asks me for my 'family name', refers to my maiden name or my new married name?

Thanks for the reply. I am a bit confused as to when family name means married name (my actual family name according to my social security card etc.) and when it means maiden? The example forms on here seem to suggest that family name always means married name unless specified, except for that one example on G-325A at the bottom?

Not sure yet, I only just sent it in. I guess we'll see.

I just looked at the example form on visajourney and you are right, it did use maiden name in the heavy box at the bottom. That doesn't make sense to me because it uses the married name in the box at the top which asks for "family name", just like the box at the bottom.

Also the I-485 first asks for family name which I put down as married name, then at part 3a it specifically asks for your name "exactly as it appears on your I-94". I feel like if they had wanted maiden name on the G325A then they would have specified it.

Anyway, the box asks for alien registration number as well, and as you say both names appear in the box above, so I should hope that either way they will figure it out.

Family name is whatever name you now go by. if you go by your married name THAT'S your family name. If you go by maiden, then THAT'S your family name. Unless it asks for your maiden name you are now forever known as married name (excluding divorce etc)

Vanessa - I think the confusion here is why the sample form puts maiden name in the box when it asks for "family name" at the bottom of G325A for AOS, it makes more sense to use married name.

It does? Okay just checked. it SHOULD be married name. Please ignore the error. I will report it.
