I also payed 50$ a couple of days ago.... just be patient when you call the doctors.... most of them try to talk you into getting a complete new medical :no: It took me about 6 calls to find a doctor who was willing just to fill out the vaccination supplement.

The rate seems to vary even by doctors in the same area.

Califonia and NY seem to be the most expensive from what people have said.

Usually it is around $30-$60.

Just make sure you state what you are after and tell them it is only the VACINATION SUPPLIMENT you need.

When I called it took me a while to explain this (I think they get a lot more full medicals than suppliments at most places)

The form takes about 5-10 mins to transfer and they usually seal it in an envolope (mine was anyway).

Call as many doctors on the list (close to your home) as you can. Get all the prices and go from there. I called two or three and they all wanted us to take the WHOLE medical again for $300 - $350 -- NOT!!

Then I finally found someone who knew what I was talking about and filled out the form for $100. Yeah, it was alot of money but it was nice to finally find someone who knew what I was talking about.

Remember -- it's all about the $$$$$$

When you do find someone, make sure they make a copy for you BEFORE they put it in the sealed envelope.

I would suggest you call around, the prices can vary considerably. Also, you might want to take the xray from the physical with you. When I took my wife to the CS they wanted to take another xray so I presented the one from her physical and saved myself $35. Good luck
