Are my Ducks in a Row?

Hi guys,

I am preparing my package to send out... we did our first green card with a lawyer so I'm kinda nervous preparing all these documents myself! Please help me make sure I'm doing this right. :)

1. I can't find anything on the form that says how much it is? Are there two checks to send again? One for the fee and one for biometrics?

My Evidence:

- Joint bank account statement with both our names and address

- Letters from my employer stating that my husband is beneficiary on my 401k and life insurance (do these have to be notorized? They are on company stationary) & copies of the signed beneficiary forms.

- Receipt from Orbitz showing we paid for 2 tickets to Denmark in April of this year with both our names on it

- Amex statement showing both our names and copies of the two credit cards

- Car insurance verification of coverage statements showing both our names at the same address (current policy and previous)

- Copy of our current lease showing both of us as leasees (this is a new lease from the last time we filed)

- Copy of a signed joint loan at our credit union

- Another joint credit card statement and copies of the credit cards

- Copies of our drivers licenses with same address

- IRS transcripts of our last 2 years of tax filings married filing jointly

- Picture of us together on our honeymoon cruise last year (professional photo with info about the cruise on it)

- Pictures of us together on various holidays (valentine's day & anniversary)

So, based on all that stuff... do I need to do the affidavit thing? Is there anything else you see that could be missing? We don't have any other joint finances that I can think of. The only other thing I could add is statements from credit cards and loans that are just in his name but at the same address.

Any guidance is appreciated!
