When I sent our package.. I included my 2x2 passport picture with white background (with my name & date of birth handwritten in black ink lightly on the back)I put photo in a plastic zip lock bag and taped it to a white piece of paper and put it behind the 325a form.. On the white piece of paper I wrote my name on it as well & taped the zip lock bag securely...I did the same exact procedure with Hubbyz passport picture & put his under his signed and dated 325a.. Good luck!! Best Wishes!1
