Are there instructions for these forms anywhere? I can't seem to find them and I want to make sure we're filling them out correctly.

Thank you both. It looks like everything has to be in capital letters? Also, for the educational institutions, do they mean college, or all the schools he attended, including elementary, and so on? What about if he was enrolled in an English school, even if it was just for a few months?

Just not sure about what schools they are asking for..... college, or trade schools, or every school they ever attended, or what.....
