AOS I485 vaccination supplement

hello everyone

I'm in the process of preparing documents on I-485 and come across vaccination supplement requirement. I learn that only the vaccination supplement is requested for K-1 visa so can I just send in my fiancee's vaccination documentation worksheet (Form DS 3025) that she got from Viet nam or I have to get form I-693 filled out by a civil surgeon. Any suggestion? Thanks for your help.

What YuAndDan said it is optional but my 3025 said incomplete on the bottom and I know that's not necessarily relevant but had it transcribed over to 693 to be on the safe side cost me $50. Did not get an RFE for that so its up to you to chance it

If her shots are COMPLETE at the time of filing then you can just send the DS-3025 showing the shots are complete. If she still requires shots then she will need to see a CS to have the required shots and have the old shots, and the new shots transcribed into the I-693. This is called a vaccination transcription. She does NOT require a new medical as long as it's less than 12 months since she had it, just the shots. I paid $50 for mine too. I did not want to risk the RFE and the slow down that causes.
