I'm going to fill up today G-325a (Biographic Info) but confused what to write in the box for middle name. I have two names-Maria Theresa. My fiance said that I should be writing only "Maria" in the box for the first name, instead of "Maria Theresa." Then, what I should be filling up in the space for middle name is "Theresa," which is my second name. I questioned him why "Theresa" has become my middle name, when it's supposed to be my mom's maiden name. He said the US Immigration does not consider the maternal maiden name as the middle name. If you have two names, your secondary name is considered as the middle name, and not your mom's maiden name. Is this true in the US?

Forumers, please help on this. I don't want to be denied in the end due to error in the middle name. Any advice would be greatly appreciated. Thank you very much and Merry Christmas to all.

i dont either,but i have a question ( sorry OP for highjacking your thread. ) if there is no middle name is it alright to leave the space for it blank or should we write N/A?

05/26/2014 - 3 year anniversary.

06/02/2014 - Interview. PASSED!

09/11/2014 - Oath Ceremony.

09/16/2014 - Updated driver licence and registered to vote.

10/08/2014 - Updated Social Security

10/14/2014 - Applied for U.S. Passport

10/27/2014 - U.S. Passport arrived in mail (13 days - DID NOT expedite)

10/30/2014 - Naturalization Certificate arrived in mail (was not folded)

I would go with what is on your birth certificate. Both my U.S. Citizen daughters have two first names. That was not my intention, but when the Philippine birth certificates were registered NSO listed their first and middle names as their first name, then my wife's maiden name, and then my last name. So, that is what is on their U.S. Passports, Social Security cards, Philippine Passports, etc.......SO, I would suggest being consistent with what is on your birth certificate.

Interesting, the actual hospital did the same with my daughter. I wrote the name on the form the way I wanted it and they made me change it so the wife's maiden name was the middle.

11/15/10: I-130 package FEDEX'd to Chicago Lockbox

11/15/10: NSO Marriage and Birth Certificates available for pick-up at NSO

11/17/10: Receipt Date of I-130 petition at Chicago Lockbox

11/19/10: NSO Marriage Cert and Birth Cert (4x each) received by Gina in Philippines

11/19/10: CRBA package couriered to US Embassy in Manila

11/22/10: CRBA package/application including NSO BC & MC received by embassy

11/22/10: NOA1 Date

11/24/10: Electronic notification of receipt received from Chicago Lockbox

11/24/10: Embassy scheduled CRBA appointment for 12/21/2010

11/26/20: Check cashed

11/27/10: NOA1 Hardcopy received via USPS

12/21/10: Interview/Personal appearance at Manila Embassy for CRBA **approved**

01/03/11: CRBA and US Passport for daughter received by Gina via FEDEX

If you have no middle name, answer either "no middle name" or "none".

If your name is Maria Theresa, then state that. Your name does not change because you move to the US and "immigration" makes no judgements about names.

Thanks for the quick replies. What's on my birth certificate is Maria Theresa and "D" is my middle initial or my mom's maiden name. Maybe, if I will write there my complete first names, I have birth certificate and other records to prove I'm "Maria Theresa."

One thing, when I try to write my name "Maria Theresa," it will not fit on the box. The space is good for one name only. I guess I have to write my name "Maria," and then my second name "Theresa" as middle name. This is really what my fiance want me to do. So confusing.

Your name is your name and should be correctly entered as in your passport. The agencies involved see every possible naming convention in the world. You need not conform your name entries to any new custom in order to immigrate to the USA.

It should be consistent with your passport. Some passports would show the family name as DABANDAN VEGA, with Maria Theresa as the given names. In that case, show Maria as first and Theresa as Middle with DEGANDAN VEGA as the family name. How does your name show on your passport?

If you have no middle name, answer either "no middle name" or "none".

If your name is Maria Theresa, then state that. Your name does not change because you move to the US and "immigration" makes no judgements about names.

"None" is an actual name in more than one language. I recommend entering "----" when there is no middle name. Enter the name as shown on the passport.

write your name this way:

first name: Maria

second name: Theresa

last name: Vega

other names: Dabandan

because here in the US, their middle name is the second name given you (if you have one, which in your case you do). if you had only maria as your name then here in the US it would just be maria (first) no middlename dabandan (last). and then when you get married, your name will be maria (first) theresa (second) your husband's name (last)

write your name this way:

first name: Maria

She's Filipina, not American. Her name needs to be entered as it is shown on her passport. Without knowing how it appears on her passport, we cannot properly advise her.

If your name doesnt fit in the box, just handwrite it instead of typing. If it still doesnt fit, you can use a seperate sheet. I would agree with the consensus of writing it how it appears on your passport.
