Hello everyone,

I hope someone can help me with this. I am currently holding the K-1 visa and filing my paperwork for the I-485 (AOS) I was reading multiple forums regarding the I-693. I completed my medical in canada in july 2010 of this year and I have my vaccination record from them as well the (DS form). As i recall, since it has been less than a year the instructions state that I do not need to get another medical done and that the civil surgeon has to transfer the vaccination form info to the I-693 form and submit a I-693a supplement form which they have to the USCIS.

Now I was also told it should be $25 average to do that.. but then most places charge $125+ because they wont sign it unless they complete the medical.

Does anyone know from the list of the USCIS website. Which civil surgeons in southern california that dont charge an outrageous price to sign their name?

I paid $75.00 ... I had to go twice. Once to get turned away and then the second time I brought the USCIS printout of the instructions for the civil surgeon. They don't even need to fill most of it out, just check the K-1's passport and then look at the vaccination record you brought with you. Call around, you'll find one that knows what they are doing eventually.

Hello everyone,

. . . and submit a I-693a supplement form which they have to the USCIS.

There isn't an I-693A supplement anymore. Immunization is all part of a newer version of the I-693. You should download the form and prepare Part 1 with your personal information, then take the form to them. Having the instructions where it says you don't need a full medical might come in handy.
