We were just about to send our AOS packet in when we realized we didn't have our DS-3025 form. I found the receipt stating the vaccionations performed and an invoice of his medical from when we applied for the K1. They even gave him a little travel medicine & vaccination booklet. My husband looked at the form online and said he'd never seen it before. Could it have been in the sealed envelope we were instructed to give the border guards? Anyways, we never recieved it, what can we do?

Also, I read it is not dire to send the packet in before his I-94 expires, is that true?

Thank you

you need to take the vaccination book to the civil surgion in your state and bring with you the I-693 and have them transcribe the vaccinations to the I-693. they will seal it in an envelope do not open it and put it in the aos package.

try calling different CSs in your state because they will try and rip you off by over charging or saying you need a full medical.

no you do not need to file before the I-94 expires, however it is in your best interest to do so ASAP. Main reason being protecting your status.
